Please ensure the following documents are enclosed before submitting the medical reimbursement claim forms at Health Centre.

• Reference letter
• Prescription
• Cash Memo
• Hearing aid verification and certification
• Glass verification and certification

In case the prescription is not enclosed or cash receipts are incomplete the bill will be processed for payment only in respect of available documents.
No supplementary claims will be entertained.
